2.Submit the original and photocopy of the following materials:
1) The list of legal representatives or main persons in charge of medical institutions and the heads of various departments and relevant qualification certificates, practice certificates, employment certificates, graduation certificates and other qualification documents;
2) Rules and regulations for the management of TCM clinics;
3) Proof of clinic site;
4) Legal person ID card and real estate certificate.
3.Review whether the materials comply with the "Regulations on the Administration of Medical Institutions" and the "Detailed Rules for the Implementation of the Regulations on the Administration of Medical Institutions".
4.Whether the on-site inspection meets the "Basic Standards for TCM Clinics".
5.Review whether it meets the conditions for start-up, and if it meets the requirements, it will be filed and issued with the "Record Certificate of Traditional Chinese Medicine Clinic".
II. Review and Decision.
1.Review whether the materials are true and complete.
2.Whether the on-site inspection meets the "Basic Standards for TCM Clinics".
3.If it meets the conditions for start-up, it will be filed and issued with the "Traditional Chinese Medicine Clinic Record Certificate".
3. Issuance and service.
1.The applicant picks up or mails the "TCM Clinic Record Certificate".
2.Delivery method: pick-up or mail.
