The 12-hour time difference, the frequent hurricanes, and the proximity of the volcano are difficulties that Wu Dexi had heard about before he set off. After actually arriving, the lack of materials that this "post-80s" did not experience when he was a child was still beyond his imagination. On the first day, I reported to the China-Dominica Friendship Hospital in Roseau, the capital of Dominica, and the roof that was torn off by the hurricane a year ago has not been repaired, and all the medical staff can only huddle into a dilapidated wooden room next to it.
The two most important things in the eyes of Wu Dexi are difficult to cook without rice here. Dominica lacks medical treatment, some medicines are gone, and there are many common domestic medicines in Dominica that are not available at all. The cardiac ultrasound machine used in the largest general public hospital in the region was purchased more than ten years ago.

After studying hard for 20 years and traveling 30,000 miles across the ocean, Wu Dexi did not come to Dominica to write a referral form. This heavy helplessness stung him deeply.

The difficulties in life were described by Wu Dexi as "just having enough to eat", and he was more concerned about the difficulties at work.
When I first arrived in Dominica, the local doctors looked polite, but they did not fully recognize the Chinese doctors in their hearts, "you need to work with them, see patients, and solve practical problems, and slowly get their approval".
At that time, the hospital did not open a cardiovascular specialty, and Wu Dexi did not have a special consultation room, so he could only "work on the move", pushing a trolley, standing on the countertops everywhere, and writing medical records for patients.
Fortunately, the breakthrough came a little earlier than he expected. In the second month after arriving in Dominica, Wu Dexi received a special patient - Martin Charles, the ambassador of Dominica to China. The ambassador suddenly felt unwell when he returned to Duoduo on a business trip and was initially diagnosed with acute gastritis by a local doctor, but Wu Dexi believed that subtle changes in the electrocardiogram supported myocardial infarction during the consultation, and administered medication for the symptoms**.
Wu Dexi's judgment was right. After his condition stabilized, Ambassador Charles went to the First Affiliated Hospital of Sun Yat-sen University for coronary intervention**, and every time he returned to Dominica, he would go to Wu Dexi's outpatient clinic for follow-up.
