At 17:00 on December 3, 2019, the welding joint of the No. 1 anaerobic tank of the No. 1 anaerobic tank of the sewage treatment station of a printing and dyeing enterprise in Haining cracked, and the tank collapsed to the north, hitting the No. 5 standard workshop of the adjacent enterprise, causing part of the building to collapse. The sewage in the tank was dumped into the workshop on the first floor of the No. 5 and No. 3 standard workshops, causing several workers working in the factory to be hit by the collapsed building and cloth shelves. The impact of sewage caused the river embankment on the west side of the tank to collapse, and the two power poles on the river embankment and the steam pipe on the other side of the river were broken by the sewage impact. The accident caused a total of 10 deaths, 3 serious injuries, and direct economic losses of about 45 million yuan.
[Cause of the accident].
1.Direct Cause:The accident anaerobic tank has not been formally designed, and the construction and manufacturing quality cannot meet the requirements of the relevant national standards and specifications for steel welded storage tanks, and there are problems such as insufficient wall thickness and strength of the tank and serious lack of welding. In the process of more than 2 years of use, the medium infiltrated and corroded at the weld seam of the welded joint of the 6th tank section and the 7th tank section, the strength continued to weaken, and the crack broke under the action of water static pressure, and then the crack developed instantaneously along the circumferential and longitudinal welds, tearing the tank body, and finally leading to collapse.
2.Indirect causes:(1) The enterprise carries out project construction in violation of regulations: the No. 1 anaerobic tank has not been formally designed by a qualified unit;On October 28, 2016, the "zero land" technical transformation project started the filing and approval procedures, but the construction of the No. 1 anaerobic tank had started in July 2016, and there were violations of construction without approvalWithout prior approval, the tank occupies land outside the red line of the land;In violation of the regulations, it was put into operation without organizing the completion and acceptance by itself.
2) Confusion in enterprise safety management: Illegal contracting of No. 1 anaerobic tank to units that do not have relevant construction qualifications for construction;Failure to carry out settlement test, hydrostatic test and non-destructive testing test in accordance with the requirements of the specification;Knowing that the No. 1 anaerobic tank was bulging during the water test, it was put into operation before it was completed and accepted after reinforcementThe safety distance between the No. 1 anaerobic tank and the No. 5 standard plant was insufficient, and no effective measures were taken to eliminate major safety hazards.
3) The design and construction unit does not have the professional contracting qualification of environmental protection engineering, and contracts the tank construction in violation of regulations;There is no relevant qualification for tank design, and it is illegal to help draw tank design sketches;Welding construction is not carried out in accordance with the requirements of the specification;Failure to implement the quality and safety record system such as construction process and test as required.

Enterprises should fully implement the main responsibility for safety production in accordance with the requirements of the "Work Safety Law of the People's Republic of China" and the "Notice on Comprehensively Strengthening the Work Safety Responsibility System for All Employees of Enterprises", and establish a safety production risk management and control responsibility system of "responsibility at all levels, everyone is responsible, and everyone is responsible".Extract safety production expenses according to regulations, strengthen personnel training, and ensure that equipment and facilities are intact and effective;Enterprise safety management personnel, key post personnel, team leaders and front-line employees should strictly perform their own safety production duties, strictly abide by the post safety operation procedures, and ensure that all employees' safety production responsibilities are in place.
2. Enterprises should pay special attention to the potential safety risks in the transformation and management of environmental protection facilities in accordance with the requirements of the "Environmental Protection of the People's Republic of China" and the "Safety Production Law of the People's Republic of China", and do a good job in risk investigation to eliminate illegal design and constructionThe facilities that are constructed and put into use in phases in the project shall be accepted in phases, focusing on the implementation of safety production responsibilities in key links such as design, construction and installation, and completion acceptance.
3. Enterprises shall, in accordance with the requirements of the "Work Safety Law of the People's Republic of China" and the "Regulations on Emergency Response to Production Safety Accidents", formulate and revise the production safety emergency rescue plan of the unit in a timely manner, improve the emergency response measures for accidents, and strictly prevent all kinds of production safety accidents.
