A panel of experts reviewing Boeing's safety management procedures released a report on the 26th, arguing that there is a "decoupling" between Boeing's management and employees in the company's safety culture.
In 2018 and 2019, the Boeing 737 MAX 8 was involved in two major air crashes, killing 346 people. The FAA appointed the panel when the U.S. Congress called for a review of Boeing's safety management procedures in 2020.
According to Reuters and Agence France-Presse, the panel found that Boeing had "deficiencies and confusion" in the implementation of its corporate safety culture. The report identifies 27 concerns and makes 53 recommendations, including standardizing safety training and improving transparency in handling employee complaints.
The FAA said it would "immediately begin a full evaluation of the report and determine the appropriate next steps to take on the recommendations made."
The report summarizes the safety measures Boeing has taken in the wake of the two crashes, such as the creation of a system called Freedom of Expression** that allows employees to anonymously report concerns about production quality, safety and business ethics. However, some employees who were interviewed by the expert group expressed distrust of the system and preferred to discuss issues directly with their managers. The report also said that Boeing employees did not know "when and which reporting system to use."
The scope of the report's review does not include the near-miss incident on a 737 Max 9 airliner in January, but the report notes that recent "serious quality issues" have "deepened" the panel's concerns.
On January 5, shortly after takeoff, a 737 MAX 9 of Alaska Airlines fell off a door plug (built-in emergency door) on the side of the cabin, but fortunately turned back safely.
