According to data from the Changsha 120 Emergency Center, from the Chinese New Year's Eve to the eighth day of the first lunar month, 71 drunken calls were accepted this year, mainly from 20 o'clock to 1 o'clock the next day. Based on the experience of previous years, the whole New Year is the peak time for receiving drunk calls.
In medicine, drunkenness is called acute alcoholism. Deng Maolin, director of the emergency department and deputy chief physician of Changsha Hospital of Traditional Chinese Medicine (Changsha Eighth Hospital), said that since February 2, drunk people will "visit" every day, and more than 10 people with severe acute alcohol poisoning will be treated in one night. Many people think that it will be fine to sleep when they are drunk, but in fact, the wrong sleeping position just buries a huge hidden danger to the life safety of drunk people. This was the fourth case of drunken patients that night, and he was 40-year-old Mr. Ma.
That night, after having dinner with his relatives, Mr. Ma rushed to attend a high school friend's party. I haven't seen my classmates drink very well for a year, Mr. Ma usually drinks a good amount of alcohol, and he drank more than a bottle of liquor at two consecutive dinners that night, and finally fell asleep on the sofa in the box.
At around 9 o'clock, the other two classmates who had drunk too much took a taxi home, and Mr. Liu, a classmate, had planned to drive Mr. Ma home, only to find that he was drunk, unconscious, and vomiting frequently. With the help of the hotel staff, Mr. Ma was lifted into the car, and Mr. Liu decided to go to the hospital first.
After the medical staff on duty in the emergency department had a preliminary understanding of the situation, they hurriedly pushed the flat car to Mr. Liu's car, opened the car door, and a pungent smell of alcohol came to their faces. I saw Mr. Ma, who was lying on his back in the back seat of the car, his face was bruised, he couldn't breathe, and he was covered with vomit.
The medical staff quickly lifted the patient onto the flat car, kept the head on the side of the decubitus, and sent him to the emergency room, where he quickly assessed the condition and monitored vital signs. The patient was in a deep coma due to drunkenness, incontinence, cyanosis at the ends of the limbs, a large amount of vomit in the mouth, shortness of breath, oxygen saturation as low as 70% under high-flow oxygen, shock blood pressure, and his life hung by a thread.
Endotracheal intubation, ventilator, drug awakening, massive infusion of anti-shock, gastric lavage ......The results showed diffuse lesions in both lungs, which were considered to be aspiration pneumonia, and the patient was transferred to the emergency intensive care unit after intubation.
After 17 hours of active rescue and meticulous care, at 3 p.m. the next day, the patient finally woke up and turned his life around.
Expert interpretation. Prevention of aspiration in intoxicated people is a top priority.
Aspiration is the leading cause of accidental death in drunk patients. Deng Maolin said that drinkers often have a large amount of food in their stomachs, and when they are drunk and vomiting, the stomach contents can easily enter the airway, resulting in suffocation and inducing aspiration pneumonia; Mild (simple drunkenness) has only neurological manifestations of emotional and verbal excitement, whereas severe alcoholism presents with coma with shock, malignant arrhythmias (frequent premature contractions, atrial fibrillation or flutter, cardiac arrest, etc.), and severe acidosis (pH 7.).2) Hypothermia, etc., which is life-threatening. Therefore, for intoxicated people, preventing the occurrence of aspiration is a top priority.
When someone around you is drunk, pay attention to help them untie their ties, neck buttons, etc.; Turn your head to one side and tilt back a little to keep your airway open. Lie on your side and add a pillow or barrier to your back to avoid turning over into a supine position during sleep. Keep warm during the care process. The caregiver should always look for abnormalities in the drunk person until he or she is fully conscious. Once it is found that the drunk person has symptoms such as coma, he should be sent to the nearest hospital in time without delay.

1.Supine position. After being drunk and full, sleeping on your back may lead to the reflux of undigested food in the stomach, and the body's swallowing function becomes sluggish under the stimulation of alcohol, and vomit is easy to block the trachea, resulting in poor breathing, which can cause aspiration pneumonia in mild cases and suffocation death in severe cases.
2.Prone position. When the mouth and nose are facing down in the prone position, the mouth and nose of the drunk person may be buried in the futon or soft pillow, resulting in poor breathing passage, and in severe cases, the body can be deprived of oxygen, and even a drunken coma. In addition, excessive alcohol consumption accelerates blood circulation, urine output increases, and when the urine is full and the bladder is higher than the pubic position, the prone abdomen is squeezed or shaken, which can easily cause bladder rupture, which is life-threatening.
3.Seat. In severely drunk patients, the cardiovascular reflex adjustment ability is weakened, and the body loses a lot of water due to vomiting and sweating, and the blood volume in the blood vessels is relatively insufficient. If you fall asleep while sitting, it is easy to cause orthostatic hypotension and even shock.
4.Sleep on your pillow arm. Sleeping on the arm for a long time can lead to compartment syndrome, which can quickly develop into limb necrosis or gangrene if left untimely.
5.Limbs press against each other. If a drunk person falls asleep with one limb pressed against the other, muscle avascular necrosis will occur at the site of long-term compression, resulting in rhabdomyolysis, which is manifested by soy sauce urine and a significant increase in creatine kinase in the blood test. Mild rhabdomyolysis resolves gradually within a week or two, but severe cases can lead to acute renal failure due to extensive muscle necrosis due to myoglobinuria.

3.If the patient is conscious and cooperative, ask the patient to bend over and guide the patient to gently stimulate the throat with his fingers to induce vomiting, or he or she can repeatedly induce vomiting with warm water.
4.If the patient is unconscious, the head should be lowered to the side to keep the airway open, and the vomit in the mouth and nasal secretions should be cleaned up in time to prevent suffocation caused by aspiration.
5.Prevent patients from falling and other injuries caused by alcohol.
6.If there is no improvement after the above treatment, or if you have severe alcohol poisoning, call 120 for emergency treatment immediately**.
