Check "Yes" in Schedule 9 in the "Summary of Attached Table Information", click "OK" in the display box to enter the senior management change operation page, fill in the relevant information of the person in charge and legal representative after the change (* is required information), and do not "modify or delete" if there is no change information;Upload the relevant attachments, and be sure to click Save after confirming that they are correctAfter saving, click "Print" to export the change application form and required schedules, print, sign, scan and upload the corresponding location of "Attachment Materials".After the export is complete, click "Submit Request" to submit the change task.
Note: The corresponding information in the profile of the application for completed and submitted tasks will not be updated, and the updated content will be displayed in the application profile information when the next task is created when the task of changing senior management is completed.
Second, the person in charge of technical quality is changed
If the technical person in charge or the person in charge of quality changes, please change it by yourself in the system: "Menu" - Maintenance User Information" - Maintenance Organization Information - Structured Self-Maintenance", and do not need to submit other materials. For specific operations, please refer to the "Notice on the Use of the Self-Maintenance Function of Laboratory Institutions" in the notification announcement on the home page of the system, and click "**" for specific instructions.
At present, only testing and calibration laboratories (lxxxxx) have this function, and other types of institutions also need to send electronic changes (change instructions, appointment documents, etc.) to [email protected] filing. Please name the subject of the email as "Change of Quality Manager or Technical Manager + Registration Number".
Note: If the change of authorized signatories is involved, please also follow the precautions related to the change of authorized signatories.
