Efficacy and safety of secondary curved fibrous ureteronephroscopic holmium laser. Clinical data and methods: There were 20 cases in this group, including 8 males and 12 femalesAge 21-63 years, average 44 years;There were 9 cases of left stone and 11 cases of right stoneThere were 4 cases of upper and 10 cases of middle stones6 cases of inferior calculus;Stones are a minimum of 08cm, up to 12cm, average 11cm;Among them, there was 1 case with isolated kidney. All patients were treated with continuous epidural anesthesia with lithotomy position.
The ureter was dilated with a 14 16fx35cm ureteral dilation sheath, and the circumeliteduro-8 secondary curved fiberoptoureteronephroscope (made in the United States, the maximum bending angle was 315°) was placed through the dilating sheath, and after peeking at the stone, a holmium laser fiber (200um) was inserted, and the beam was nearly 70° from the surface of the stone.
The crushed stone power is 5-10hz, and the energy is 06-1.0j。Results: There were 20 cases of holmium laser lithotripsy ** renal calyceal stones in this group, all of which were successful, with a single operation stone crushing rate of 100%, an average operation time of 80 minutes, an average stone removal time of 2 weeks, and an average hospital stay of 5 days. There were no complications such as ureteral perforation and tear during the operation. There were no complications such as infection, fever and obstruction after surgery. Conclusion: Holmium laser lithotripsy with secondary curved fiberoptic ureteronephroscopy is an efficient and safe method for alyceal stones, and its efficacy and safety are significantly higher than those of ESWL and other in vivo lithotripsy (especially suitable for 1Calyceal stones of about 0cm), which is an ideal lithotripsy technique. Intraoperative stone displacement is mostly related to surgical procedures. The use of ureteral dilation sheath significantly shortens the time required for endoscopy and is also beneficial for the protection of fiberoptic ureteronephroscopy (as opposed to nurethery).
