1. Information required for applying for environmental pollution control capacity evaluation certificate.
1.Basic information of the applicant unit;
2.Approval of the environmental impact report (form) of the construction project;
3.Registration card for environmental protection acceptance application for completion of construction projects;
4.Registration form of the applicant for personnel engaged in environmental impact assessment;
5.Registration form of the applicant for the environmental protection acceptance, monitoring or investigation of the completion of the construction project;
6.Annual work report (including technical report, management report, monitoring report) of the operating unit of environmental pollution control facilities;
7.A copy of the qualification certificate (copy) of the operating unit of the environmental pollution control facility;
8.An appraisal report issued by a nationally recognized credit rating agency.
2. Information required to apply for a Class A certificate for the operation of environmental pollution control facilities.
1.Basic information of the applicant unit;
2.The applicant's experience in the operation of environmental pollution control facilities and related achievements;
3.Approval of the environmental impact report (form) of the construction project;
4.Registration card for environmental protection acceptance application for completion of construction projects;
5.Registration form of the applicant for personnel engaged in environmental impact assessment;
6.Registration form of the applicant for the environmental protection acceptance, monitoring or investigation of the completion of the construction project;
7.Annual work report (including technical report, management report, monitoring report) of the operating unit of environmental pollution control facilities;
