[Editor's note].

Since 2010, when the National Development and Reform Commission, the Ministry of Education, the former Ministry of Health and other five departments launched the first batch of rural order-oriented free medical students (hereinafter referred to as "order-oriented medical students") training work, 13 years have passed, and batches of medical students have gradually returned to their hometowns and grown into the "mainstay" that can provide general medical services for the people. In order to tell the stories of order-oriented medical students from all over the country to study, return to their hometowns, and practice medicine, the "People's Good Doctor" client launched the "Rural Order-Oriented Free Medical Students Series Stories" to show their style of taking root at the grassroots level and serving the people.

Recently, three order-oriented medical students from Guangxi Zhuang Autonomous Region told the "People's Good Doctor" client their story of going from medical students to doctors, and then serving the grassroots.

The first batch of declarationsLead by example and guard the "door" of grassroots health

Chen Jia, vice president of the Central Health Center of Xiling Town, Gongcheng Yao Autonomous County, Guilin City, Guangxi Zhuang Autonomous Region, was born in 1991 in a rural area under the jurisdiction of Xiling Town. When he was a child, the underdeveloped medical and health conditions in the towns and villages, and the inconvenience of the villagers to see a doctor, planted a seed of studying medicine in his young heart. "In 2010, when I was 19 years old, I learned that the state carried out free training of rural order-oriented medical students when I filled in the college entrance examination, so I applied for the examination and became the first batch of rural order-oriented medical students in the country, and since then I have embarked on the road of studying and practicing medicine. Chen Jia said.

The picture shows Chen Jia (second from left) leading medical staff to conduct ward rounds for inpatients.

After five years of bachelor's degree and three years of residential training, Chen Jia returned to work in Xiling Town Central Health Center in July 2018. When I first arrived, there were 12 doctors in Xiling Town Central Health Center, and only 6 of them had practice certificates, and most of them had secondary school degrees. The environment is backward, the medical technology is weak, and the satisfaction of the masses is not high......Many problems appeared in front of Chen Jia, who was "new to the workplace".

In the face of difficulties, we must have countermeasures. According to the comrades in charge of the relevant work of the Guangxi Health Commission, in order to increase the performance guarantee and strive to let the targeted medical students "stay", the Guangxi Health Commission optimizes the post allocation and enrollment mechanism according to the actual work of the targeted medical students in the township health center after graduation, establishes a green channel for the recruitment, guarantees the treatment of the targeted medical students in a timely manner, and makes every effort to do a good job in job placement. With the help of the Science and Education Publicity Department of the Guilin Municipal Health Commission, the Gongcheng County Health Bureau and other parties, Chen Jia formulated a development plan for the health center serving the health of the grassroots people.

After the expiration of the service period in July 2021, a higher-level medical institution extended an "olive branch" to Chen Jia, but Chen Jia gave up this opportunity and decided to stay at the grassroots level. Today, under the promotion of Chen Jia, Xiling Town Central Health Center has been comprehensively upgraded in terms of overall planning, performance target assessment, medical quality management, and medical environment, and the level of medical services is also constantly improving.

I was born and raised here, carrying my original intention, and also accumulating my rich feelings, and I must insist on being a grassroots 'health gatekeeper'. Chen Jia said.

Health science, focus on pre-illness and weave a dense grassroots public health "network".

Ou Xiamian from Linluo Village, Dutong Town, Sanjiang Dong Autonomous County, Liuzhou City, Guangxi Zhuang Autonomous Region, became an order-oriented medical student in 2013 and returned to the Central Health Center of Fulu Miao Township in 2021.

The picture shows Ou Xiamian (first from left) coming to replace the gastric tube for a patient with sequelae of cerebral infarction who has been paralyzed in bed for a long time.

Soon after returning to the health center, Ou Xiamian was transferred to the Department of Public Health as the head of the department. As a family physician, Ou Xiamian combines the "prevention of disease" concept of traditional Chinese medicine with the "primary prevention" concept of Western medicine, and is committed to keeping diseases away from the masses and preventing them before they occur. According to Ou Xiamian, members of the public health department of the Fulu Miao Township Central Health Center need to provide physical examination services and health education services for residents, as well as health management guidance for patients with chronic diseases such as hypertension and diabetes.

Due to outdated concepts, there are still some elderly people who are reluctant to participate in health check-ups, refuse to take blood and do electrocardiograms. The elderly think that it is good to have no physical examination, and they need to take medicine if they find problems after physical examination. Ou Xiamian said that the resistance of the masses to the health examination service made her feel the necessity of informing residents of the role of physical examination and popularizing health knowledge to the grassroots people. She also knows that the change of perception is not something that happens overnight, so she has strengthened her belief in continuing to serve grassroots health. "Everyone is the first person responsible for their own health, and they have a health responsibility for their family and society. Popularizing health knowledge and improving the health literacy level of the whole people is one of the most fundamental, economical and effective measures to improve the health level of the whole people.

In the targeted medical student training project, the government and the autonomous region finance invest a lot of funds every year to support, with the aim of strengthening the construction of primary medical and health personnel and improving the capacity of primary medical and health services. It is of great significance to improve the level of medical services, promote the sinking of medical resources, and build a reasonable medical treatment pattern of 'primary diagnosis, two-way referral, acute and slow treatment, and hierarchical diagnosis and treatment'. The comrade in charge of the relevant work of the Guangxi Health Commission said.

White-clothed armor, overcoming complications, and completing the "affairs" of grassroots medical affairs

Diagnosis and treatment of common diseases, management of chronic diseases, identification of critical and critical patients, pre-hospital first aid and referral ......General practitioners undertake the important mission of protecting the health of the grassroots people and play an important role in basic medical and health services.

Mo Yongjin is part of the targeted medical student training program that has returned to Guiping City, Guangxi Zhuang Autonomous Region. When he came to Xiawan Town Health Center in 2019, he took care of the work of internal medicine, surgery, pediatrics, emergency, public health, preventive health care and other clinical front-line departments. He described the primary health center as "the primary medical work is many and complicated, and every day we have to face various clinical diseases, like 'opening a blind box', and general medicine comes in handy." ”

The picture shows Mo Yongjin (first from right) going to the village to carry out first-aid skills and other knowledge science education.

According to Mo Yongjin, the work at the grassroots level requires more patience and has accumulated more experience in dealing with the people on the front line. "In addition to completing the daily clinical work, I will also arrange time to go to the village with my colleagues from the Department of Public Health to carry out the contracted service of family doctors with the masses, and conduct physical examinations for the masses and answer the physical examination reports in detail; He also accompanied the Preventive Health Department to the village to do a good job of vaccination; At the same time, we regularly arrange time to go to the village to carry out health knowledge popularization, such as first aid for drowning, home treatment of traumatic people, self-management of common and chronic diseases, and training on how to use the Heimlich method for airway foreign body obstruction. In Mo Yongjin's view, the enthusiasm of the masses for learning Xi and interest in the content of health science popularization are the biggest motivation for him to carry out health science popularization work as a grassroots doctor.

On how to continue to further promote the training of targeted medical students, the relevant work of the Guangxi Health Commission comrade in charge said: in the future, the first is to strengthen the coordination of medical education, strengthen the organic connection between college education and post-graduation education, take the cultivation of clinical post competency as the core, improve the training system during the college curriculum and standardized training, strengthen the education of medical ethics and medical style, clinical thinking, clinical practice and scientific research and teaching ability, especially the cultivation of general practice ability, so that it can adapt to the needs of primary medical and health services and become oneThe general practitioner of "good treatment of minor diseases, good knowledge of major diseases, good treatment of serious diseases, and good management of chronic diseases" ensures that targeted medical students can "use and do well". The second is to continue to improve the supporting system conducive to the landing of talents and professional development, and give policy preferences in terms of establishment, personnel hiring, title promotion, on-the-job training, award and promotion, etc., so as to smooth the career development channels of general practitioners, improve the career attractiveness of general practitioners, and ensure that targeted medical students "can go down and develop".
