Qingxi Hospital, Qishi Hospital

The two hospitals are at a major milestone in their development

Successful transformation to:

Have the service capacity of county-level traditional Chinese medicine hospitals

A new type of town-level traditional Chinese medicine hospital

How can grassroots hospitals develop and grow?

How in a medical institution.

Stand out from the competition?

Facing the needs of the masses.

Combined with the "Millions of Projects".

and the requirements for high-quality development of health and health.

Dongguan has explored a new path.

In the Linshen area of Dongguan, Qingxi Hospital has developed Dali and No. 3 Middle School Hospitals into traditional Chinese medicine hospitals, with a 100% rate of traditional Chinese medicine outpatient clinics and 99% patient satisfaction, establishing a good reputation and making traditional Chinese medicine services more accessible.

Qingxi Hospital and Qishi Hospital have been successfully transformed into new town-level traditional Chinese medicine hospitals with county-level traditional Chinese medicine hospital service capabilities.

Such a transformation initiative initiated in the country not only focuses on solving the "embarrassment" of not having a county-level traditional Chinese medicine hospital in the "Zhizhuzi City", but also makes a unified layout of the regional traditional Chinese medicine medical center, but also creates a new sample for the transformation of grassroots hospitals, and firmly supports the "bottom of the network" of traditional Chinese medicine services.

Activate the new situation of the development of traditional Chinese medicine at the grassroots level

Traditional Chinese medicine is a treasure of Chinese civilization and an important carrier of excellent traditional culture.

In the past ten years, China has consolidated the grassroots Chinese medicine service network and strengthened the leading role of county-level traditional Chinese medicine hospitals. By the end of 2020, 1,615 counties had set up county-level traditional Chinese medicine hospitals, accounting for 86% of the country's county-level hospitals. The diagnosis and treatment volume of county-level traditional Chinese medicine hospitals accounts for about 70% of the total diagnosis and treatment volume of county-level traditional Chinese medicine medical institutions.

Dongguan City is one of the four prefecture-level cities in the country without counties and districts, and there are no subordinate districts and counties. Although there are tens of millions of people and trillions of GDP, the city only has three public traditional Chinese medicine hospitals, Dongguan Hospital of Traditional Chinese and Western Medicine, and Humen Hospital of Traditional Chinese Medicine.

However, at the same time, among the 42 public hospitals in Dongguan, most of the town-street public hospitals are in the "sandwich layer", facing disadvantages such as homogenization, and some have operating losses, and transformation and upgrading have become more urgent.

Most town-street hospitals face common problems: unclear positioning, weak service capacity, and idle medical resources. Affected by salary and platform, there is a lack of sufficient talents and technical support for business development, high-level talents cannot be introduced or retained, and it is difficult to invest in high-end fine equipment. Zeng Xianhui, secretary of the general party branch of Qishi Hospital, said frankly that the hospital must closely follow the requirements of the supply-side structural reform of public hospitals in the whole town street in order to catch the express train of high-quality development.

In the view of Huang Junjie, president of Qingxi Hospital, transformation is also an inevitable move to "break through". **The "siphon" effect of hospitals, the medical requirements of local residents to "leave the town when they are seriously ill", and the development needs of attracting and retaining high-level talents make town-level hospitals more need to improve their service capabilities and develop in dislocation.

How can the general public get more equitable, accessible, systematic and continuous TCM services nearby? This is a problem that Dongguan urgently needs to solve. The implementation of the "Millions and Thousands Project" has put forward higher requirements for Dongguan's health work.

Combined with preliminary research and expert demonstration, according to the city's population distribution and the medical needs of the masses, Dongguan took "Chinese and Western coordination, optimization of layout, structural adjustment, and building a solid grassroots" as the reform idea, and selected Qingxi Hospital and Qishi Hospital, two town-street public hospitals with a good foundation of traditional Chinese medicine, and successfully transformed into a new town-level traditional Chinese medicine hospital with the service capacity of county-level traditional Chinese medicine hospitals.

Coupled with the already vigorous development of Humen Hospital of Traditional Chinese Medicine, there are now a total of 3 new town-level traditional Chinese medicine hospitals in Dongguan, which has basically eliminated the blank area of traditional Chinese medicine resources and accelerated the expansion and balanced layout of high-quality medical and health resources. The relevant person in charge of the Dongguan Municipal Health Bureau introduced.

Exploring the construction of a new town-level traditional Chinese medicine hospital equivalent to the service level of county-level traditional Chinese medicine hospitals is not only the first in the country of "Zhiguanzi City", but also an important embodiment of Dongguan's implementation of the "Millions of Projects", comprehensively deepening the comprehensive medical reform in the new stage, and breaking through the development problems of town-level hospitals, which will provide the province and even the whole country with a replicable Dongguan experience, and promote the inheritance, innovation and development of traditional Chinese medicine.

Find the positioning

"Small hospital".A "big jump" in a short period of time

The characteristics of the new town-level traditional Chinese medicine hospital are that it has the service capacity of county-level traditional Chinese medicine hospitals, reasonable regional distribution, and can form a pattern of Chinese and Western coordination, complementary functions and dislocation development with other general hospitals.

In recent years, Dongguan City's traditional Chinese medicine service system has been continuously improved, from the traditional Chinese medicine hall and national hospital of the city's leading hospitals to the traditional Chinese medicine pavilion and traditional Chinese medicine pavilion of primary medical and health institutions. However, due to the inability to set up county-level traditional Chinese medicine hospitals and the lack of a strong and powerful "waist" part, the city still has the problem of insufficient and uneven distribution of traditional Chinese medicine resources.

The construction of a new town-level traditional Chinese medicine hospital is vividly called the "waist-strengthening" measure of Dongguan's traditional Chinese medicine service system.

The crux of the problem of the development of traditional Chinese medicine in our city lies in the 'deficiency of middle qi', that is, the development orientation of traditional Chinese medicine in Zhenjie Hospital is not clear, the advantages and characteristics of traditional Chinese medicine are not highlighted in the process of operation and management, and the 'waist section' of the traditional Chinese medicine service system is relatively weak, which cannot play a pivotal role in connecting the upper and lower levels. The relevant person in charge of the Dongguan Municipal Health Bureau said.

The "Notice of the Dongguan Municipal Committee of the Communist Party of China and the People of Dongguan City on Promoting the Implementation Plan for the Inheritance, Innovation and Development of Traditional Chinese Medicine (2021-2025)" issued in 2021 proposes to encourage the transformation of public general hospitals in towns (streets) with a good foundation of clinical disciplines of traditional Chinese medicine and qualified towns (streets) to traditional Chinese medicine hospitals.

In 2022, Dongguan City was identified as a pilot for Guangdong Province to build a national demonstration zone for comprehensive reform of traditional Chinese medicine with the theme of "building a 15-minute high-quality traditional Chinese medicine service circle". The "Pilot Work Plan for Dongguan City to Promote the Construction of a National Demonstration Zone for the Comprehensive Reform of Traditional Chinese Medicine in Guangdong Province (2023 2025)" clearly states that combined with the layout of the city's existing public traditional Chinese medicine hospitals, in areas where traditional Chinese medicine medical resources are relatively lacking, 2 3 town-level general hospitals with a good foundation of traditional Chinese medicine will be transformed into traditional Chinese medicine hospitals or integrated traditional Chinese and Western medicine hospitals, so as to solve the problem of no county-level traditional Chinese medicine hospital traditional Chinese medicine service platform and resources.

In 2023, experts will conduct many field investigations and demonstrations to find out the foundation, positioning, and planning direction for the transformation. Zhang Qiaoli, director of the Dongguan Municipal Health Bureau, led a team to coordinate with the local towns and hospitals for many times, and the town and street health, land planning and finance departments worked together to study the feasibility and transformation model of hospital upgrading and transformation. During the period, the "Implementation Plan of Dongguan Medical Security Bureau on Medical Insurance to Support the Inheritance, Innovation and Development of Traditional Chinese Medicine" was promulgated, which promoted the increase of the number of dominant diseases of traditional Chinese medicine with DIP score payment from 38 to 74, and the dominant diseases of traditional Chinese medicine requiring long-term hospitalization, such as palliative care, medical treatment, and psychiatry, were determined according to the bed day.

The city's game of chess, the joint construction of cities and towns, and the innovative management model of "three doses of medicine" at the same time, the "qi stagnation and blood stasis" in the reform have been opened up one by one. At the end of 2023, the transformation plan will be finalized, and a key step will be taken to build a new town-level traditional Chinese medicine hospital. Through the construction of the municipal-level platform and the guaranteed investment of the town-street, the service and operation capacity of the hospital has been rapidly upgraded.

After the transformation, the number of public hospitals of traditional Chinese medicine in Dongguan has increased to 5, which are evenly distributed in the urban area, Songshan Lake area, Binhai area, southeast Linshen area, and eastern industrial park area, which effectively "supports" the city's traditional Chinese medicine service system.

Among them, the number of beds in Humen Traditional Chinese Medicine Hospital, Qingxi Hospital and Qishi Hospital is 500, 665 and 448 respectively, with a radiation radius of 2 million, 1.3 million and 800,000 people; The talent echelon of the three hospitals is reasonable, with 8, 3 and 3 key (characteristic) specialties of traditional Chinese medicine respectively, all of which have appropriate technology promotion centers for traditional Chinese medicine, and have a strong cultural atmosphere of traditional Chinese medicine.

According to estimates, based on the data of TCM diagnosis and treatment volume and proportion of town-street hospitals in 2022, after the construction of a new town-level TCM hospital, the number of TCM diagnosis and treatment in town-street hospitals in the city will increase from the original 1.09 million to 2.14 million, accounting for 810% to 16%.

This powerful measure will allow the total amount of traditional Chinese medicine services in Dongguan to "double", the medical resources of traditional Chinese medicine to grow, and the city to form a new situation of relatively balanced and coordinated development of regional Chinese and Western medical and health levels.

Nowadays, the city's traditional Chinese medicine medical resources are expanding incrementally, Dongguan to build a new town-level traditional Chinese medicine hospital as the basic, to promote the expansion and quality of high-quality traditional Chinese medicine resources, balanced layout and sinking communities, so that the public can walk 15 minutes to obtain high-quality traditional Chinese medicine services, the "millions of projects" have achieved initial results.

Seek new solutions

There is a new way out for the development of grassroots hospitals

In Qingxi, 81-year-old Mr. Li was urgently admitted to the hospital for "9 days after thoracic and abdominal aortic reconstruction, and myocardial enzymes were found to be elevated for 7 days" and was in critical condition. Yan Fang, director of the traditional Chinese medicine classic ward of Guangdong Provincial Hospital of Traditional Chinese Medicine, who came to teach, decided to use a large dose of extraordinary heart-saving soup after treating the old man. After taking the medicine, Mr. Li's condition gradually stabilized and he was discharged from the hospital in good health.

Traditional Chinese medicine is simple, convenient, inexpensive and very well recognized by the masses. Zhang Zesong, director of the Department of Traditional Chinese Medicine of Qingxi Hospital, is the most beautiful scientific and technological worker in Dongguan City and the leader of the characteristic specialty discipline of traditional Chinese medicine in Dongguan City, and he is deeply touched by the results brought by the transformation. Today, there are more than 100 inpatients in many first-class wards of the hospital, and famous doctors and departments have attracted more surrounding people to seek medical treatment.

According to the requirements of national construction, each county-level traditional Chinese medicine hospital must build "two specialties and one center", that is, two specialties with traditional Chinese medicine characteristics and one county-level traditional Chinese medicine appropriate technology promotion center. On the basis of these hardware conditions, the three new town-level traditional Chinese medicine hospitals have drawn up a development blueprint and taken a new step on the road to breakthrough.

Qingxi Hospital started earlier. In 2022, the hospital became a cooperative hospital of Guangdong Provincial Hospital of Traditional Chinese Medicine, and in 2023, a working group on the integration of traditional Chinese and Western medicine composed of medical, nursing, personnel, finance and other relevant departments of the hospital was established, and remarkable progress has been made in the construction of talent team, department allocation, diagnosis and treatment technology, promotion of traditional Chinese medicine culture, and practice of preventive treatment.

In the first half of 2023, all middle-level cadres of Qingxi Hospital carried out a warm demonstration meeting, which had a clearer layout of the current situation and future of the hospital, and a clearer goal of transformation.

The hospital will pay attention to the inheritance and development of the classic connotation of traditional Chinese medicine, and apply for provincial and municipal continuing education classes in the category of traditional Chinese medicine, and continue to carry out research on new methods and technologies for the prevention and treatment of traditional Chinese medicine. In terms of the scale of the first integrated traditional Chinese and Western medicine hospital, the hospital plans to further build itself into a non-directly affiliated hospital of Guangdong Pharmaceutical University, improve its scientific research and academic capabilities, and strive to become the "flagship" integrated Chinese and Western medicine hospital in Dongguan.

The infrastructure of Qishi Hospital is good, and the prospect of the integration of traditional Chinese and Western medicine is optimistic. To this end, the hospital plans to strengthen the construction of the Chinese medicine talent team, select 5-10 business backbones to study in the superior Chinese medicine hospital within 5 years, set up a famous doctor studio, and plan to introduce 10-20 graduates of traditional Chinese medicine. At the same time, the hospital also encourages clinical doctors to "learn from the West", and strives to improve the theoretical and practical level of traditional Chinese medicine of doctors at all levels in the hospital.

Chen Rongzhuang, the president of Qishi Hospital, who studied under Wei Guikang, a master of traditional Chinese medicine, is the leader of the science department of traditional Chinese medicine and the inheritor of the sixth batch of municipal intangible cultural heritage "Lingnan Guyuan Moxibustion Method" in Dongguan City. He introduced that by 2026, the number of TCM physicians in the hospital will reach more than 30% of the total physicians, so as to achieve full coverage of TCM physicians in all clinical departments of the hospital, and all clinical departments of the hospital will be able to provide high-quality TCM services to patients.

In 2024, Qishi Hospital and Dongguan Hospital of Traditional Chinese Medicine will establish a close medical alliance of traditional Chinese medicine in Dongguan in the form of "hospital and hospital co-construction", open 500 beds, conduct a grade review of second-class first-class traditional Chinese medicine hospitals, and improve the level of traditional Chinese medicine medical services in the hospital.

Positioned as a "Marina Bay Boutique Traditional Chinese Medicine Hospital", Humen Traditional Chinese Medicine Hospital has passed the grade evaluation of second-class A traditional Chinese medicine hospital in 2022, and started construction of a new comprehensive building in October 2023, which is expected to be completed in 2026, and cooperated with Guangdong Pharmaceutical University to build it. At that time, the construction area of the hospital will reach more than 80,000 square meters, and the number of beds will reach 800, reaching the scale of a first-class hospital, and a comprehensive traditional Chinese medicine hospital with beautiful environment, complete equipment, efficient informatization and convenient medical treatment for the masses will be built, and the radiation will serve the 3.5 million population of the Guangdong-Hong Kong-Macao Greater Bay Area.

Such supply-side structural reform has allowed grassroots hospitals to break through the "sandwich layer" dilemma, enhance their competitiveness, and win the reputation of the masses. Today, Qingxi Town is fully promoting the new hospital project, with a total investment of about 1.5 billion yuan and 1,200 medical beds. After completion, the hospital's management and service capabilities will be significantly improved, "We will strive to let the people enjoy high-quality medical resources without leaving the town." ”
