Mechanics and inflammatory treatment of knee osteoarthritis

Mondo Health Updated on 2024-03-03

Knee osteoarthritis is a chronic degenerative bone and joint disease characterized by degeneration, destruction and bone hyperplasia of knee cartilage, which is mainly manifested as joint pain, swelling, deformation and dysfunction. As the saying goes, people grow old first, and with age, the knee joint is caused by cumulative damage, and with the aging of China's population, the incidence of knee osteoarthritis is on the rise.

There are two main factors that cause knee pain, one is inflammatory and the other is mechanical. Inflammatory problems can be alleviated with medications** and mechanical factors can be relieved with knee orthoses.

Squeezable extremity pressure bands to draw effusion** inflammation.

The most typical clinical symptoms of knee arthritis are pain and swelling, and swelling generally represents joint effusion caused by abnormal synovial function of the knee joint, which usually produces more severe pain or discomfort.

When there is a large amount of fluid and tension in the joint, a joint puncture can be performed to completely evacuate the fluid and blood. Therefore, the clinical treatment method for the onset of knee arthritis is generally to eliminate the cause of swelling, extract the fluid in the knee joint, reduce the pressure of the swelling on the knee joint capsule, and then inject analgesic and anti-inflammatory drugs into the joint, and inject joint lubricant after the water accumulation is controlled to replace the function of the extracted joint fluid.

Traditionally, the doctor and one or two assistants squeeze the patient's knee capsule, patella, popliteal fossa, etc. with their bare hands to drain the effusion. This method has the following problems: waste of medical human resources and physical consumption; The manual extrusion efficiency is low and the operation is time-consuming; Unable to accurately control the extrusion pressure, incomplete extrusion; Relying on the operator's experience to judge the pressure in the knee joint, there is no quantitative standard as a reference, and each operation is not replicable.

The team of Academician Dai Yurong and Professor Wang Jinwu of the Ninth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine has invented a negative pressure controlled squeezable limb pressure band to assist in the extraction of knee joint effusion, which can quantify the pressure exerted on the knee joint, make the negative pressure in the joint cavity more accurate and quantifiable, and the suction effect of the effusion is more stable, which is basically comparable to the arthroscopic joint irrigation effect, and the time is short, basically non-invasive, low cost and reliable effect.

The knee joint is squeezed by the deformation caused by the inflation of the annular balloon, so that the fluid in the joint cavity can quickly gather together.

3D printed knee orthosis solves the problem of alignment.

Skewed alignment of the lower extremities is another cause of knee pain, and the knee space often narrows on one side and the femoral and tibial heads wear and tear increases, resulting in knee pain.

Among the non-pharmacological modalities, the use of knee orthoses in the early and middle stages of knee osteoarthritis is the most important means long before surgery.

The team of Academician Dai Yurong and Professor Wang Jinwu from the Department of Orthopedics of the Ninth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine proposed an improved single (double) reducer orthopedic principle on the basis of the overall reducible orthopedic principle, and designed a single (bi) reducible knee orthosis. By applying axial tensile force of different magnitudes on both sides of the knee joint, according to the principle of action and reaction force, the principle of lever balance and the principle of force decomposition, a corrective moment in the direction of the side with less force is generated, and the local load is reduced on one side (both sides) while correcting the force line and reducing the load on the affected side of the patient, so as to avoid aggravating the wear and tear of cartilage.

On this basis, the team also applied 3D printing technology to the design and manufacture of knee orthoses, and developed a new type of personalized 3D printing single (double) side reload-reducing knee orthoses, which not only reduced the weight of the orthosis but also greatly improved the comfort of patients wearing the orthosis.
