This article is from "Nankai Sun Pharmacist", which is used for medical science popularization for reference. Dexzopiclone is the S-type isomer of zopiclone, so it is also known as eszopiclone, which belongs to the new non-benzodiazepine sedative-hypnotic drug, is a fast-acting sleeping drug, which mainly acts on central benzodiazepine receptors, and selectively strengthens the central inhibitory neurotransmitter - aminobutyric acid to bind to its receptor, enhances the effect of -aminobutyric acid, has sedative-hypnotic and muscle relaxant effects, but the binding mode with benzodiazepine receptors is different from benzodiazepines (diazepam). Dexzopiclone has a fast onset, a long time of action, few adverse reactions, can significantly shorten the latency period of falling asleep, reduce the number of night awakenings and early awakenings, improve sleep quality, hypnotic efficacy and benzodiazepines are comparable, rarely hangover, low risk of drug dependence, mainly used in clinical insomnia, especially for insomnia patients with difficulty falling asleep and sleep maintenance disorders, and is currently the first choice for insomnia in clinical practice.

Dexzopiclone should be taken before going to bed, high-fat foods can affect its absorption, therefore, should be avoided with high-fat foods, the recommended starting dose is 2mg, if the effect is not good, the dose can be increased to 3mg, the starting dose for the elderly is 1mg, if necessary, it can be increased to 2mg, and the maximum dose of a single dose does not exceed 75mg, if you wake up during the night and are more than 5 hours away from the expected time of getting up, you can add one tablet of dexzopiclone, and the continuous use of dexzopiclone will not have an accumulation effect. In addition, if the patient is prone to early awakening and has difficulty falling asleep after waking up, medium- and long-acting sleeping pills such as Apu ** and Ace** can also be selected, but these drugs are not as safe as dexzopiclone.

In order to exert the efficacy of the drug and avoid adverse reactions, the following principles should be followed when taking dexzopiclone:

1.Start with a small dose and gradually increase to the lowest effective dose, avoid continuous medication every night, and take the drug as needed, intermittently, and for a short period of time, and it is recommended to take the drug 2-4 times a week to avoid premature tolerance.

2.The duration of taking the drug should not exceed 4 weeks, and if it is taken for more than 4 weeks, it should be re-evaluated by a doctor to avoid mental dependence.

3.Patients who have been taking the drug for a long time should gradually taper the dose to avoid abrupt discontinuation and prevent rebound insomnia and withdrawal syndrome, which should continue for a few weeks to several months.

Dexzopiclone has a strong hypnotic effect, and it is taken according to the prescribed dosage and principles, and has good safety and tolerability, but long-term use may produce the following harms:

1.Gastrointestinal reactions: bitter mouth is the most common adverse reaction of long-term use of dexzopiclone, and its incidence can reach 15%-30%, because the bitter mouth is due to the fact that dexzopiclone can be secreted from saliva and has a high concentration, most patients can tolerate it, and it does not affect the continuation of the drug. Dry mouth is also an adverse reaction with a high incidence of dexzopiclone, and other gastrointestinal reactions include nausea, vomiting, diarrhea, constipation, epigastric pain, and dyspepsia.

2.Drug dependence: long-term use of dexzopiclone is prone to drug dependence, generally 6-8 hours after taking the drug, the desire to take the drug again, abrupt stop the drug will cause withdrawal symptoms, manifested as nausea, vomiting, anxiety, nightmares, agitation, night sweats, tachycardia, myalgia, tremor, insomnia rebound, etc.

3.Psychiatric and nervous system reactions: Long-term use of dexzopiclone can show drowsiness, anxiety, nervousness, depression, headache, dizziness, irritability, vertigo, agitation, fatigue, nightmares, insomnia, tremor, memory impairment, neuralgia and residual sedative effect of the next day.
