5. A copy of the label (packaging) of the controlled item or its layout certified by the applicant;
6. Sampling behavior (sampling);
7. Manufacturer's (manufacturer's) statement on genetically engineered genetics, nanomaterials, hormones, and pesticides;
8. Research (testing) protocols (health professional knowledge behavior), scientific reports, and expert opinions.
9. The applicant is responsible for the reliability of the documents provided for the issuance of product (goods) safety confirmation documents.
List of registration documents for foreign-made personal and oral hygiene products
1. Application; 2. Copies of documents (standards, technical conditions, regulations, technical descriptions, specifications, formulas, and ingredient information) on the basis of the production of personal hygiene products, certified in accordance with the laws of the country where the country is registered;
3. Manufacturer's (manufacturer's) statement on whether it contains genetically modified organisms, nanomaterials, hormones, and pesticides;
4. Documents (instructions, manuals, regulations, suggestions) or copies certified by the applicant (if any) of the manufacturer (manufacturer) on the application (operation, use) of personal hygiene products;
5. Written notice from the manufacturer (manufacturer) that the personal hygiene products it produces meet the requirements of the production documents;
6. A copy of the manufacturer's (producer's) document certifying the safety and quality of the samples tested, certified in accordance with the legislation of the Contracting Party that carried out the national registration;
7. A copy of the product label (packaging) certified by the applicant;
8. Copies of documents certified by the competent health authority of the country producing personal hygiene products (authorized bodies of other countries) in accordance with the legislation of the Contracting Party that carried out the national registration, confirming the safety of these products and allowing them to circulate freely in the territory of the country where the manufacturer (producer) is located, or information provided by the manufacturer that does not require such documents;
9. Research (experiment) program (health professional knowledge behavior), scientific report, expert opinion;
10. A copy of the certificate of import of the sample of personal care articles into the customs territory of the Customs Union, which shall be certified in accordance with the law of the Contracting Party in which the State is registered.
11. Foreign language translations of manufacturer's (producer's) documents must be certified in accordance with the laws of the Contracting Party in which the state registration is carried out.
