Primary medical and health institutions are the "first line of defense" to protect people's health. The report to the 20th National Congress of the Communist Party of China stressed the need to develop and strengthen the medical and health workforce, focusing on rural areas and communities.

Only by cultivating an excellent team of primary medical and health talents, so that grassroots hospitals can attract and retain more young talents, can the people truly seek medical treatment at home with peace of mind, and no longer "run to big hospitals for major and minor diseases".

When she brought her mother to Dr. Zhang Lei in the community hospital, Li Ying actually didn't have much hope in her heart, her mother had been suffering from lymphoma for 10 years, "I just want to try again".

For the past few years, she has been battling cancer cells for her mother, "with chemotherapy every six months." Every time I take my mother to a tertiary hospital for re-examination and chemotherapy, it is like fighting a "battle" that makes people physically and mentally exhausted. At the beginning of last year, my mother had a low-grade fever and pain all over her body, and her weak body could no longer withstand the "heavy blow" of chemotherapy, and the old man's mental state became unstable and began to resist**.

In desperation, Li Ying was recommended by a friend and came to Zhang Lei to try it with traditional Chinese medicine. What she didn't expect was that after taking the medicine for about a month, she felt that her mother's condition had improved, and half a year later, "she grew flesh and slept better". More importantly, the doctors here often comfort and encourage the elderly, so that the elderly can rekindle their hope for life.

Our Chinese medicine may not cost much money, and we can cure the disease, or in other words, live with the disease. As the director of the Department of Traditional Chinese Medicine at the Dongxin Street Community Health Service Center in Hedong District, Tianjin, Zhang Lei is good at explaining the principle of Chinese medicine treatment in the vernacular, "coexistence means that no one is messing with anyone."

It's hard to imagine that this community hospital, located in an old neighborhood, is changing the traditional concept of many residents "running to the big hospital no matter how big or small it is". The TCM Inheritance and Innovation Studio here has been rated as a famous doctor studio, and it is a team of young doctors with an average age of about 30 years old. In the past year, the annual number of consultations of this young team has been close to 50,000, which confirms that famous doctors are not only evaluated, but also the choice of ordinary people to "vote with their feet".

Banners sent by residents were hung all over the walls, and Zhang Lei often reminded the young doctors in the team to empathize and thank the patients for their trust, "This is a profession that entrusts one's life."

To attract talents, we must also cultivate and retain them.

Early every morning, the entrance of the Chinese Medical Hall on the second floor of the Dongxin Street Community Health Service Center is full of residents waiting to see a doctor. Many people come from other districts.

Accompanied by the fragrance of traditional Chinese medicine, 31-year-old physician Liang Shuaihu began a busy day, consultation, massage, cupping, orthopedic ......He has been working here for 10 years, and many of the residents in the neighborhood are his old friends.

After graduating from Tianjin University of Traditional Chinese Medicine, he was one of the few among his classmates to work in a community health service center. When he first came here, he was taken aback: he didn't expect the humble community hospital to be so popular and so many patients to see him.

At that time, there was no online appointment system, and the hospital opened at 7 a.m., and there was already a long queue at the door. Liang Shuaihu recalled that there were few doctors in the Chinese medicine department at that time, and he saw Zhang Lei busy from morning to night in the small consultation room, and he was often so busy that he didn't have time to eat at noon, and he always greeted patients with a smile when facing patients, "He was very serious about seeing patients, and he was very patient and meticulous with patients. ”

Liang Shuaihu speaks slowly, with a gentle and kind appearance, he also has his own fan base, and many people often recommend relatives and friends to come to him for consultation and prescription because they trust him. Some patients sighed, "When I see this young man, my heart is steady." This gives him a special sense of accomplishment.

Some patients made a special trip from Inner Mongolia. Liang Shuaihu said: "The common people are running for good doctors. We have to work hard to take care of all the patients. To be a doctor, you must first have a benevolent heart. ”

Previously, many medical school graduates were reluctant to work in grassroots hospitals, mainly because they were worried that the grassroots treatment was not high, patients were not recognized, and the growth space of doctors themselves was limited. But in the process of dealing with patients, Liang Shuaihu really felt that the choice of patients fundamentally depends on whether there are "good doctors" in "small hospitals".

With good doctors, there are naturally more people coming to see the doctor, which forms a virtuous circle. In Zhang Lei's view, grassroots hospitals should not only bring in professional talents, but also find ways to train and retain them.

The young doctors in the team have to participate in various levels of training and professional seminars every year to see the world in practice, develop skills, and constantly improve their all-round capabilities.

The 28-year-old majored in traditional Chinese medicine, acupuncture and massage, and when he first came here to work after graduation, he was also worried, "At first, I felt that in terms of personal development, small hospitals are definitely not as good as big hospitals." After he came, he found that "the level of the grassroots hospital is also quite high." He often has the opportunity to study in different tertiary hospitals, and often participates in some high-level conferences, and the large number of patients admitted by the team also provides him with rich cases for research topics and publications.

The young doctors in our team are the best of the best, growing very fast, and all of them can take charge of themselves. Zhang Lei said.

The in-depth collaboration of medical alliances makes it more convenient for patients to see a doctor.

Unlike large hospitals that are overcrowded, in community hospitals, doctors and patients are closer to each other, and they can get along for a long time and build trust with each other.

Auntie, where are you uncomfortable? Sit down, I'll show you. Liu Chaona, a young doctor with a small height and a ponytail, spoke like a person, crisp and neat. The 28-year-old girl is the only woman on the team. In addition to consultation, she mainly does medicine jars, acupuncture, acupuncture injections and other work in the physiotherapy room, "many aunts like to look for me", she said with a smile, the work intensity is quite high every day, and she has to constantly "recharge" when she gets home from work, she likes this kind of work state.

For doctors, it is necessary to keep pace with the times, learn new knowledge, and continue to innovate. "Primary care doctors need to have the competence of general practitioners. Zhang Lei said. In the daily diagnosis and treatment work, the needs of patients are different, and he has led the team to carry out more than 10 diagnosis and treatment projects such as bone injury massage, acupoint injection, puncture and bloodletting, scraping physiotherapy, neck and waist traction, ancient medicine cupping, Tongdu cupping, acupoint dressing, and internal and external washing of traditional Chinese medicine.

After more contact, he empathized with the pain of patients, "We do not prescribe large prescriptions, and try to make the treatment course as short, quick and effective, low cost, and do not increase the burden on patients."

In recent years, Dongxin Street Community Health Service Center and the tertiary hospital have established a medical alliance, and now, on the basis of the original hierarchical diagnosis and treatment system of "primary diagnosis, two-way referral, acute and slow treatment, and upper and lower linkage", the community health service center has further opened a direct inpatient green channel from the tertiary hospital.

The medical alliance mechanism can guide the orderly sinking of high-quality medical resources, create a full-chain continuous medical service, and make more patients more willing to come to the hospital at their doorstep. Zhang Lei said that grassroots hospitals can solve the problem of minor diseases and common diseases, and if they encounter some really complex diseases, they can also directly connect with the top three hospitals to make an appointment for hospitalization, which makes up for the problem that some grassroots hospitals do not have inpatient departments, and simplifies the admission process so that patients can be treated in a timely manner.

When the patient's acute and critical illness is relieved, the primary hospital at the doorstep can continue to provide the patient with follow-up family doctor health monitoring and guidance services. Zhang Lei said that his mobile phone must be charged at any time, "I don't know when patients will come to me." When they are not feeling well, or their family members want to see a doctor, they will ask me on WeChat. He has nearly 5,000 WeChat friends, of which more than 4,000 are his old patients, "Now the elderly over 60 years old and patients with limited mobility in the community, as long as they call a **, we will provide door-to-door services." ”

Be a little closer to the common people.

In order to do a good job in primary medical services, the team has set up a volunteer service team of the Orthopedic Massage Center, prepared to build a health lecture platform for traditional Chinese medicine, and continuously improved the technical level of clinicians.

No matter how busy she is, Han Lu, a 30-year-old young doctor, regularly goes to the community and nursing homes with her team to participate in volunteer activities such as free clinics. He graduated from Tianjin University of Traditional Chinese Medicine, and when he was in school, the "great medical sincerity" proposed by the famous ancient medical scientist Sun Simiao was deeply imprinted in the heart of this young man. These four words are known as the "Hippocratic Oath of the East", and they always remind those who practice medicine not only to have superb medical skills, but also to have good medical ethics. After joining the work, she came to the grassroots hospital and got along with the patients like relatives, and Han Lu felt the deep meaning of this sentence more and more.

He has been practicing medicine for more than 20 years, and in Zhang Lei's heart, the most important quality of a good doctor is to serve the people wholeheartedly, "just like we honor our parents, we don't think about anything in return."

He always felt that he was getting too much in return for his efforts. On the walls of the corridor, there are honorary plaques such as "Famous Doctor's Studio" and "Tianjin Youth May Fourth Medal", "The society has given us a lot of honors, and we must find ways to better serve patients."

In 2020, the team opened WeChat*** to share TCM health knowledge in an easy-to-understand, relaxed and happy way. Liang Shuaihu undertakes the creation and release of first-class content, and in his spare time, he will carefully sort out patient cases, common problems, etc., and make them into popular science shorts, so that more people can know health knowledge.

He noticed that in recent years, the proportion of young and middle-aged patients has gradually increased, and many diseases come from poor lifestyle habits. "A lot of patients don't realize that these small habits in life can lead to big problems. Liang Shuaihu said that from this point of view, the role of health education is even greater than that of many patients by learning practical exercise methods such as cervical spine exercises, so that the disease has been alleviated. ”

Because of his superb medical skills, Zhang Lei, who was born in a grassroots hospital, is still practicing in a tertiary hospital in Tianjin. Over the years, some hospitals have thrown olive branches to him, but he still chooses to stay in the grassroots hospital he loves, and together with the group of young people in the team, "get closer to the people".
