How the accident happened:
Before the Guangzhou Office Environment Exhibition, Foshan Jingyi Company contacted Guangzhou Xinwei Design Engineering and Guangdong Zhongshi Company to be responsible for the design and drawing and construction of the development decoration project; Guangdong Zhongshi Company contacted Dongguan Rongchangda Company to assist in the construction of the development position. From March 23 to March 27, 2023, Dongguan Rongchangda Company arranged personnel to go to the Canton Fair Complex to set up a development position. On March 31, Zheng * and others from Dongguan Rongchangda Company arrived in Guangzhou, and went to the event in the evening to remove the lamps and lanterns. At about 8:30 on April 1, Zheng *ping and Zhang*, Li *tian, Wang *cheng, Li *ming (deceased) and others arrived at Area D 20 of the Canton Fair Complex2. At about 11 o'clock before the incident, the on-site operation was dismantled to the sloping ceiling. Zhang * dismantled the inclined ceiling connecting bolts (a->b->c) in a clockwise direction, and Li * Tian removed the inclined ceiling connecting bolts (d->c) along the counterclockwise direction of the truss. There is a material elevator at the bottom of the sloping ceiling to support the sloping ceiling.

At about 11:50, Zhang * and Li * Tian worked near point C, and Li * Ming (the deceased) and Wang * Cheng waited below the edge of the sloping ceiling to move the ceiling. After disassembling a connecting bolt and adjusting the position to prepare to disassemble the adjacent connecting bolt, Zhang* stepped on the sloping ceiling with his right foot, causing the ceiling to tilt and slide, hitting Li *ming, who was standing below. After the accident, the on-site operators immediately launched rescue. After 120 ambulance personnel arrived at the scene, Li *ming was sent to the 74th Group Army Hospital of the Chinese People's Liberation Army for rescue. At 7:18 on April 2, Li *Ming died after rescue efforts failed. 
Analysis of the direct causes of the accident after the on-site operation situation:Workers work in violation of the Operational Safety Rules. Li Ming stood beneath the sloping ceiling. At the time of the incident, Li *ming was standing under the sloping ceiling (the load of the material hoist), in violation of the relevant provisions of the "Operation Safety Rules" for the hazard warning of smashing. Zhang * stepped on the sloping ceiling with unreliable connecting bolts. At the time of the incident, Zhang * was working on the ceiling in the processThe right foot is on the sloping ceiling where the attachment bolts are being removed, which violates the relevant provisions of the "Operational Safety Rules" to avoid falling hazard warnings. The sloping ceiling is not fixed to the fork frame and the load center is not maintained in the material lift load center area. Before the incident, when the material elevator was supported in the center of the inclined ceiling load, the area of the inclined ceiling was much larger than the area of the fork frame of the material lift, and according to the principle of levers, the ceiling can be balanced when the ceiling load around the fork frame is equal. When Zhang* stepped on the inclined ceiling, the load center of the inclined ceiling deviated from the load center area of the material elevator, and the inclined ceiling lost its balance on the fork frame of the material elevator and rolled over due to unreliable connecting bolts and failure to fix the inclined ceiling to the fork frame, which violated the load positioning instructions in the "Operation Safety Rules". In October 2023, the investigation determined that the 4·1 "general object strike accident" of Rongchangda Exhibition Service in Dongguan City, Haizhu Pazhou was a general production safety liability accident.
